Establish general practice’s ‘diagnosis gap’ and local trajectories for improvement.
Review patient lists: vulnerable older patients in the community, and in care homes.

Where dementia is moderate to severe (i.e. established dementia)

  • check whether a diagnosis has been recorded on the General Practice dementia register;
  • record a diagnosis where indicated

DRIVER: Patients with co-morbid conditions and/or complex needs
