This project will support dementia Link nurses in West Kent to work in primary care and proactively identify, diagnose, triage and assess people with dementia and provide appropriate prescribing and post diagnostic support. Read more »
This project will support people with dementia to be discharged back to their own home as soon as possible following an attendance at A&E or an admission to an acute hospital in West Kent. Read more »
The Dementia Shared Lives Project, will seek to develop a bank of Shared Lives placements, where people open their own homes to provide care to people with dementia, these placements could be used for long term care, rehabilitation/recuperation, respite short breaks and social and day activity.
