Connecting Care across Cheshire aims to join up local health and social care services around the needs of local people and take away the organisational boundaries that can get in the way of good care.

Local people will only have to tell their story once – rather than facing repetition, duplication and confusion. Also the programme will tackle issues at an earlier stage before they escalate to more costly crisis services.

There will be a particular focus on older people with long-term conditions and families with complex needs. The joint approach will mean:

• Reducing the repetition and duplication
• No one falls through the gaps
• An end to revolving door syndrome
• Coordinated home visits
• Discharge from hospital at the right time with the right support
