The LiveWell Dementia Hub will be a single one stop shop offering assessment and information for people in Stockton-on-Tees living with dementia, their families, carers and the wider public.

In July 2013 Stockton Council and its health partners was awarded £576,546 by the Department of Health to create the pioneering centre to help support people living with dementia and their carers.

Stockton Council and Tees, Esk and Wear Valleys NHS Foundation Trust will work together to transform the Halcyon day centre through an extensive programme of refurbishment utilising dementia friendly design principles to enhance and enrich the client experience and support the provision of outstanding care for those living with dementia.
