This tool was developed to help commissioners and others with a stake in shaping local markets to understand and assess their progress in delivering personalisation in adult social care.
It reflects the renewed commitment to transformation through personalisation and community based support set out in the Government’s new vision, Capable Communities and Active Citizens and the new sector wide partnership agreement, Think Local, Act Personal.
