A new mobile application has been developed by clinicians in Plymouth, supported by international colleagues, that provides an easy-to-use dementia assessment tool that can be used by a wide range of medical and other healthcare staff. Read more »

Bracken ward in Derriford Hospital, Plymouth, has established a small activity room and volunteers have been recruited to work with people living with dementia attend the ward on a twice weekly basis to support patients in the activity room. Read more »
A screening tool used in Derriford Hospital is helping staff to identify those at risk of swallowing difficulties, enabling them to give advice and strategies for improving swallowing and when to refer for specialist assessment. Read more »

Patients admitted through Urgent Care in Plymouth hospitals are screened on admission to identify the risk of complex needs in hospital, and to predict those on discharge.
