What is being done?
The Trust has established a computer-based system to manage the delayed discharges that is reviewed daily by key members of the care team who have responsibility for progressing the patients journey. A daily update on the patients progress is input on the system.
What is new and different?
The South West Dementia Partnership Peer Review team members were impressed to see this ‘live’ system in place that is both current and owned by all members of the care team.
What difference this seems to be making
It was evident from the information captured on the system daily that progress was being made to more effectively manage potential delays. This was reinforced by the Director of Operations at the South West Dementia Partnership Peer Review feedback sessions.
Plans for taking the project forward
This approach was shared with other organisations throughout the Peer Review process as many Trusts were experiencing similar difficulties with delayed discharges.
