This project aims to support CCGs in the South West to reduce emergency admissions to hospital for people with dementia by reviewing and redesigning care pathways.

This project includes:

• With CCGs, review and benchmark HES data
• Identify and negotiate priority areas for pathway redesign
• Facilitate CCG-led projects to improve pathways and reduce emergency admissions
• Monitor and evaluate as part of continuous improvement cycle
• Disseminate outcomes, learning and positive practice
